Owner relinquishment Form

I, the undersigned, hereby state that I am the legal owner of a Spaniel named below and as such I wish to transfer ownership of the said dog to UK Spaniel Rescue with this taking effect from the date on this form

I understand that, from this date, I relinquish any ownership rights and acknowledge and accept that I will have no further claim with regard to the Spaniel in the future.

However, should there be problems within the home chosen for the dog, I understand that the dog will be returned to me. Should I refuse to take the dog it will be placed in emergency boarding and I will be charged for each night the dog is boarded.

Decisions regarding this dog will be made by the permanent owner selected by and in conjunction with UK Spaniel Rescue.

I confirm that the details I have provided are true to the best of my knowledge and I have not concealed any details regarding the safety or health of the dog.

I further declare that the dog named above is not subject to any ownership disputes or any legal proceedings.

Note: In accordance with the Data Protection Act 1998, I agree that UK Spaniel Rescue may hold and use personal information about me for rescue reasons. This information, including that contained in this document, can be stored on both manual and electronic files. This information will not be shared out with UK Spaniel Rescue. I confirm that the information on this form is correct.
